General Insurance Information
We are committed to providing you with the best possible care and establishing and maintaining a solid physician-patient relationship. Helping you understand our financial policy before your visit allows a good flow of communication and enables us to achieve our mutual goals.
The Nephrology Clinic accepts most insurance plans, including Medicare and Medicaid. Please verify with your insurance company that we are a participating provider in your network.
Please bring your insurance cards and any required referrals to all appointments. We ask that you provide this information at each visit to ensure our information is correct. As a courtesy to our patients, we will submit claims for you. Insurance coverage varies, and therefore patients are ultimately responsible to our office for all fees incurred.
Your insurance plan states you are responsible for paying all copays, deductibles, and coinsurances at the time of your visit. Copays will be collected when you check-in for your appointment. For your convenience, we accept VISA, MasterCard, Discover, American Express, personal checks, and cash. If you are seen at our Laramie, Sterling, or Fort Morgan clinics, please call our billing office at 970-221-3589 regarding payment of your copays. For our current fee schedule, click here.
Supplemental Insurance Coverage
As a courtesy, we will submit claims to your supplemental insurance company. If there is no response within 60 days, the balance will become your responsibility.
Uninsured Patients
Payment in full is expected at the time of your visit unless arrangements are made in advance. A discount is given when your bill is paid in full at the time of your visit. Please contact our billing department for our current self-pay discounts.
Collection Policy
A statement will be sent to you if necessary after your insurance pays. Any delinquent, unpaid balance will be sent to an agency for collection. We realize financial difficulties may affect the timely payment of your account. If such problems arise, please contact our billing department.
